Reconnect Care & Support Ltd

Care at home

A tailor-made care package, built around you

Our person-centred home care services are constructed around the individual and their needs — not around what is easiest to staff.

Live-in care

A carer or support worker moves in with you, so home stays home.

Our 24-hour live-in service gives you a healthcare assistant or support worker who moves into your home and provides a tailored care service, so you keep your independence and the comfort of your own surroundings. Unlike a care home, live-in care is one-to-one and adapts to your routine as circumstances change. Because sharing your home is a personal matter, we match staff to your situation and preferences as well as to any clinical needs, and brief them fully before they arrive.

Hospital to home

Support for the weeks either side of a hospital discharge.

Coming out of hospital is easier with someone alongside you. Our hospital to home service eases you back into familiar surroundings, whether you need attentive care or just daily support. It suits anyone due to be discharged, anyone about to be admitted who will need support afterwards, anyone wanting practical advice about getting home, and anyone already home who has found they need more help than expected.

Specialist care

A person-centred plan built after a full assessment, and reviewed as needs change.

We offer specialist care for adult clients who want to keep living an active, rewarding and independent life. After assessing your health and social care needs we build a comprehensive person-centred care plan around you, then review it regularly to make sure it still fits. Carers are selected in advance so the right person is ready when you need them.

Respite care

A break for you, without disruption for the person you care for.

Caring for a loved one weighs heavily, and taking time for yourself matters. Respite care keeps your family member's support in place while you are away, and is far less disruptive than temporary residential care — home routines stay as they are, in surroundings they know. It can run from a few days to several weeks, and can be arranged in an emergency if you fall ill or go into hospital yourself. It is also a good way to see what a home care service feels like before committing to one.

End of life care

Comfort for your loved one and support for the family around them.

When someone is diagnosed with a terminal illness they often want to spend their final days at home. Caring for them there is emotionally and physically exhausting. We help care for your loved one, help around the house, and give the family emotional and moral support through it.
